The University Payroll System requires a valid Social Security Card upon hire for any monetary compensation from the University.

All employees will need to complete all of the above forms and submit supporting identification that verifies identity and employment eligibility. Acceptable documents are listed in Column A, B, and C on the I-9 form.


EMPLOYMENT

Assignment/Change Form - To be completed for new hires, changes in rate of pay, FTE changes, changes in employment locations on campus, changes in position or title, supervisor changes and for changes in benefit status.

Pay By Requisition Backup Documentation

Budget (FOAP) Change Request - To be completed only when FOAPAL changes occur or for reallocation purposes.

Request for Provisional Position

Termination Workflow - To be completed for any employee leaving employment with the University.  Login to In.AState and click on the Banner tab.

Banner Security Approval Form - To be completed when an employee needs to be an approver and/or needs access to internet native Banner HR forms.

Proxy Assignment Form - To be completed when supervisor needs to assign a proxy to approve time.
