ASU Wellness Benefits
All ASU benefited employees receive $500 toward wellness check-ups. Below is the care covered by the $500.

Routine Physical Exam - PCP/in network paid 100%, no deductible, $30 copayment, benefits cover one physical exam per calendar year - maximum benefit $500 (referral requirement waived).

PSA - PCP/in-network paid 100%, no deductible, $30 copayment, (males age 40 and over), limited to one self referral per calendar year.

Routine Gynecological Exam - PCP/in-network paid 100%, no deductible, $30 copayment, limited to one self-referral per calendar year. Includes office visits, pap smear, urinalysis, hematocrit or hemoglobin and hemoccult stool.

Mammography - PCP/ in-network reimbursement rate 100%, no deductible, limited to one self referral per calendar year. Referral requirement is waived. Limited to the following frequency schedule: Age 35 through 30, one exam; Age 40 and older, one exam per calendar year.

Flu Shot - Covered at 100%, no deductible, Flu mist not covered. Other adult immunizations not covered. Employee would be subject to a $ 30 copay.
NOTE: The following services DO NOT apply towards your $500 deductible: PSA/Occult Blood Exam/GYN Exams/Mammograms
Routine Well Baby/Well Child Care Exams

PCP & In-Network Provider - Paid at 100%, no deductible, $30 copayment. Available for children from birth through 6 - no limit. After age 6, maximum benefit $500 applies.

Routine Child Immunizations - Available for children from birth through age 18.

PLEASE NOTE: When receiving this service please tell your doctor's office you need this to be coded as WELLNESS.
This is not a guarantee of benefits. If this document conflicts in any way with the policy issued to your employer, the policy shall prevail. Providers need to verify member's eligibility via AHIN or Customer Service at 1-800-370-coinsurance.
