1. Accept your Financial Aid Perkins Loan offer by logging into Banner Self Service through mycampus.astate.edu.
2. Complete the Perkins entrance interview here.
3. Sign a prepared Perkins Master Promissory Notes and Perkins Loan Disclosure form. Perkins Promissory notes will be available in the Financial Aid and Scholarships Office each year beginning in August.  After August begins, Promissory notes will be available within 3 business days after accepting a Perkins loan.
4. The Financial Aid & Scholarships office is located in the Reng Student Services Center(Student Union), Room 2078, and are open Monday through Friday from 8:00 AM to 5:00 PM

The Perkins Master Promissory Notes and Perkins Loan Disclosure MUST be completed otherwise your Perkins Loan funds will NOT be applied to your account. It is essential that all forms be completed prior to the beginning of the Fall semester.

Remember, you will also need to complete the Perkins Loan Entrance Interview.
