EVENT FORMS
INSTRUCTIONS ON FILLING OUT THE EVENT FORM:
1) Pick an event you wish to sign up for from the list of events on the bottom of the form 2) Select the type of event: Major, Team, Tournament, or Special Event
3) Indicate whether your team or organization wants to participate in the All University Point Race ( 2 teams per organization) 4) Captain & Co-Captain ASU e-mail address, cell phone number, & whether or not you can receive text messages

5) Pick the league that your team wish to participate in:A League (No Faculty/Staff & is eligible for extramural tournaments), B League, Greek (active ASU member of IFC, NPC, or NPHC), Co Rec, Singles, or Doubles.

6) Pool play instant scheduling will be in effect for major events. Days & times have already been selected for certain leagues. During pool play, team, tournament, & special events instant scheduling will not be used. All teams & participants must be available for all times & days
6) The maximum amount of participants that a team can have is sixteen (16). To be added to a team you must have the proper name of the participant, valid ASU ID, & sign the waiver.
7) All players on your roster must read the release & indemnity section of the event form & sign next to their names. Players will not be added to the roster if this item is missing. After rosters are turned in players can be added by filling out the roster addition form. The captain of the team must sign the bottom of the roster addition form & the players being added must sign the waiver.
FIELD RESERVATION FORM
INSTRUCTIONS ON FILLING OUT THE FIELD RESERVATION FORM:
1. Request a reservation by completing the appropriate Facility Reservation Form and returning it to the Intramural Sports Office in HPESS Room 136. All information requested must be complete, including event description, equipment needed, signatures, etc. before reservation requests may be considered. 2. A copy of a valid Arkansas driver's license or ASU ID is required with each reservation.
3. Approval of reservation requests is by the Coordinator of Intramural Sports only. 4. Pay the deposit to reserve the amount of fields & any additional fee's
5. All reservations must be made at least 48 hours in advance. No reservations will be made the day of the event. Campus Recreation reserves the right to cancel a reservation for an outdoor facility in the event of inclement weather
