Good afternoon.

During the regular course of business on August 26, 2010, a report containing the First Name, Last Name, Social Security Number, Driver’s License Number, and driving record “points” information of full and part-time employees authorized to operate State of Arkansas vehicles was sent inadvertently via email to 144 ASU email addresses by an employee in the Travel Office.

IT has been actively involved in deleting the message, tracking the forwarding of the original, and will be notifying impacted individuals in accordance with state statutory requirements.

We are writing to ask that you assist us in this matter by absolutely not forwarding, printing, saving, or disclosing this protected information in any way.  If you have received a copy of the message/file, please immediately delete the file from your email account and from your computer.  By forwarding or saving this protected information, individuals assume legal liability for that information.

The attached file was named “VEHICLE REPORT FY 02.rtf”. If you have this file in your mailbox, on your computer or phone, or have retained it in any way, we ask that you assist us by immediately deleting this information.
